The Head Start Performance Standards state that WE MUST HAVE AN UPDATED PHYSICAL/WELL CHILD CHECK UP and DENTAL EXAM on file for each child’s health record.
Dental exams are needed (and covered by Medicaid) every 6 months for children/toddlers 12 months (1 year) and older
Physicals are needed yearly for children 3 years and older (Head Start age). They are needed on a schedule for children 0 – 36 months (Early Head Start age). See Physical/Well child checkup schedule here
If you have not turned in this documentation into your child’s center, please turn it in as soon as possible.
If your child is not up to date on a Physical and/or Dental Exam, take your child for the needed physical/ well-child check or dental exam as soon as possible and turn in your child’s form to the Center Assistant/Manager or your Family Engagement Advocate.
If you have an appointment date, please tell the center’s health staff or your Family Engagement Advocate the appointment date.
